This Privacy Policy explains how Empowering DME Medical Supplies (“we,” “us,” or “our”) collects, uses, shares and protects information when you visit our website, call or text us, or order equipment and supplies from us in Georgia and Nevada.
Text messaging (SMS) and mobile information
SMS Privacy Notice: Mobile information, including phone numbers and SMS opt-in/consent data, will not be sold, rented, shared, or disclosed to third parties or affiliates for marketing or promotional purposes. SMS opt-in consent and mobile phone numbers are used solely to provide requested communications and will not be shared with third parties for their own marketing purposes. Consent to receive text messages is not a condition of purchasing goods or services. Message frequency may vary. Message and data rates may apply.
No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Text messaging originator opt-in data and consent will not be shared with or sold to any third party.
We do not sell, rent or share your mobile phone number or text-messaging consent with anyone for their marketing purposes. All of the sharing described elsewhere in this policy excludes text messaging originator opt-in data and consent.
If you opt in to text messages, we use your mobile number only to send you messages about order and delivery updates, delivery and pickup scheduling, billing and copay reminders, and replies to questions you send us. We may use a text-messaging platform provider (such as our phone and messaging service) only to deliver messages on our behalf. Those providers may not use your information for any other purpose.
Information we collect
- Contact details you give us, such as your name, phone number, email address, delivery address and the best time to reach you.
- Order and insurance information needed to supply equipment, such as prescriptions, physician orders, insurance member ID and date of birth, collected by phone, fax or in person.
- Quote and callback requests you send through our website forms, such as the equipment you need, how it will be paid and your delivery ZIP code. We do not take payments on this website.
- Technical information your browser sends automatically, such as IP address, browser type and pages visited, used to keep the site secure and working.
How we use information
- To fill, deliver, set up, service and pick up medical equipment.
- To verify insurance coverage, estimate copays and bill you or your insurance.
- To respond to callback requests and questions.
- To send order, delivery and billing messages by phone, email or text (texts only if you opt in).
- To protect against fraud and meet legal and regulatory requirements.
Health information (HIPAA)
Protected health information we receive as a medical equipment supplier is handled under the Health Insurance Portability and Accountability Act (HIPAA) and described in our Notice of Privacy Practices. None of this sharing includes mobile phone numbers or text-messaging opt-in data or consent, which are never shared with third parties or affiliates for marketing or promotional purposes.
